Practicum and Social Justice
Social justice in healthcare emphasizes the provision of fair and equitable medical services to all patients regardless of their socioeconomic status, ethnicity, citizenship, or gender identity (Habibzadeh et al., 2021). In this practicum assessment, I will describe my experience through the PICOT framework (Patient, Intervention, Comparison, Outcome, Time) focused on reducing Pressure Injury (PI) incidences among elderly patients. Additionally, I will address the challenges faced and strategies used to deliver culturally appropriate care.
Significance of Learning about Multicultural Care
Understanding multicultural healthcare is essential in improving patient satisfaction, experiences, and health outcomes. A lack of cultural awareness can lead to negative events in care delivery and affect healthcare providers’ attitudes toward culturally diverse patients (Kaihlanen et al., 2019). Effective intercultural care requires recognition and respect for patients’ beliefs, cultural norms, and backgrounds. Different cultures influence how patients interact with healthcare providers and adhere to treatment plans (Stubbe, 2020).
Patients’ socioeconomic and cultural backgrounds directly impact healthcare engagement. Healthcare professionals who employ culturally competent approaches build patient trust and communicate effectively to meet individualized needs with culturally sensitive care plans (Stubbe, 2020). In my practicum, I emphasized patient education tailored to each individual’s unique circumstances, applying evidence-based self-care strategies, such as the PI prevention bundle intervention.
Learning about multicultural healthcare also highlights disparities among ethnic groups. Awareness of these disparities is crucial since they contribute to delayed evaluations and suboptimal PI management. By identifying barriers faced by marginalized populations, healthcare providers can create tailored interventions to reduce PI rates and improve outcomes. Cultural factors further affect patients’ adherence to care and prevention guidelines (Wan et al., 2023). Recognizing that cultural beliefs influence patients’ willingness to accept specific treatments is vital (Handtke et al., 2019). For example, some patients’ self-care preferences may conflict with standard PI prevention protocols. Thus, culturally tailored interventions are necessary to enhance compliance and reduce PI complications (Wan et al., 2023).
Ethical Obligation of Nurses
Nurses have an ethical responsibility to provide culturally sensitive care, which aligns with nursing ethical principles such as autonomy, beneficence, nonmaleficence, and justice (Varkey, 2020). Culturally sensitive care considers patients’ cultural backgrounds and how their beliefs shape healthcare decisions (Stubbe, 2020). According to Ocran et al. (2022), a culturally competent environment increases the effectiveness of healthcare delivery. Nurses, as frontline care providers, must acknowledge and respect cultural differences to improve patient access, satisfaction, and experience.
Cultural competency supports the principles of beneficence and nonmaleficence by preventing harm and promoting well-being. Incorporating cultural factors into care plans enhances patient satisfaction and ensures fairness and equality in treatment delivery (Cheraghi et al., 2023). Nurses are also ethically tasked with addressing social determinants of health and eliminating disparities and stereotypes in healthcare systems. Adopting culturally sensitive practices guarantees equitable care for all patients, regardless of their social or cultural identities (Varkey, 2020).

Challenges and Accomplishments
To address the high rate of PI in elderly patients, I implemented a PICOT-driven intervention focused on PI care bundles to monitor and manage PI incidences. The aim was to provide healthcare providers with timely, evidence-based insights that allow quick adaptations in care to improve outcomes. Ensuring culturally appropriate care was a key goal, but this posed challenges such as language barriers, cultural differences, and healthcare disparities among patient groups (Kaihlanen et al., 2019).
To overcome these challenges, I developed cultural sensitivity training standards integrated with a multilingual approach. Educational materials, including posters and brochures emphasizing cultural competence, were distributed in healthcare settings. Staff training highlighted the importance of cultural awareness. During patient education sessions, I addressed individual social, familial, and ethnic obstacles patients encountered in self-managing PI prevention. Gathering patient feedback allowed me to tailor guidance based on evidence-based self-care practices, which proved vital in improving patient adherence and outcomes (McGregor et al., 2019).
Conclusion
My practicum experience working with elderly patients provided a valuable opportunity to apply the PICOT framework and evaluate the effectiveness of a PI prevention bundle. Through this work, I gained deep insight into the importance of culturally competent care within healthcare settings. Recognizing and respecting cultural diversity is crucial to improving patient satisfaction and treatment adherence. I remain committed to delivering culturally appropriate care that enhances health outcomes for patients from diverse backgrounds.
