Major Depressive Disorder (MDD) is a prevalent psychological illness that disturbs a person’s routine work and health. It is linked with numerous physical health complications like cardiovascular diseases (Cui et al., 2024). It contributes to mortality related to chronic conditions. The inclusive approach to treating MDD covers three essential dimensions: mental, physical, and medicinal. It is collectively referred to as the 3Ps. This concept map is designed to apply the 3Ps agenda within a case study context. It offers deeper insights into MDD and supports informed, complete treatment planning.

Case Study

The case centers on Ivy Jackson, a 63-year-old woman exhibiting hallmark indicators of MDD. Following a recent divorce three weeks ago, Ivy has been experiencing profound fatigue, disrupted sleep patterns, intense emotional distress, and weight loss. Although she denies any suicidal ideation, her poor nutritional habits and heightened anxiety have impaired her ability to carry out daily activities. She is currently undergoing treatment for hypertension and has a strong familial predisposition to both depression and high blood pressure, as seen in her mother, father, brother, and sister. Based on her reported symptoms and medical background, a provisional diagnosis of MDD has been made. It recognizes its impact on both her psychological state and physical health. Pathophysiology

MDD reflects dysregulation in the balance of key neurotransmitters: serotonin, dopamine, and norepinephrine. It results from a variety of genetic predispositions and adverse life events such as Ivy’s 5-year-old traumatic sexual abuse experience, which influences brain chemistry. This imbalance in neurotransmitters results in abnormal regulation and cognitive function (Cui et al., 2023).

Pharmacology

Ivy is currently prescribed sertraline to control her high blood pressure. This medication, classified as a selective serotonin reuptake inhibitor (SSRI), plays a crucial role in enhancing serotonin levels in the brain. SSRIs are widely recognized for their efficacy in treating MDD and represent a common pharmacological option in addressing her MDD symptoms (Murphy et al., 2021).

Risk Factors

Ivy’s susceptibility to MDD is amplified by a hereditary tendency to mental health issues, including a family history of depression, high blood pressure, and cerebrovascular events. These genetic loadings, combined with her past sexual abuse, divorce, and limited social engagement, lift her risk for developing and sustaining depressive symptoms (Bond et al., 2022).

Diagnostic Procedures

To validate the diagnosis and rule out other potential conditions, the healthcare provider should conduct a detailed preassessment. It includes patient interviews, administering the PHQ-9 depression screening tool, and a comprehensive physical examination (Cui et al., 2024).

Physical Assessment

Assessment indicates that Ivy is experiencing hallmark signs of depression, such as persistent fatigue, disturbed sleep, poor food, noticeable weight loss, and dismal appetite. A lack of social engagement or social interest, emotional withdrawal, and inability to express her feelings further confirm the necessity of support from her immediate family (Bond et al., 2022).

Nursing Diagnosis

Ivy’s recent emotional withdrawal and separation has triggered maladaptive coping strategies linked to her prolonged grief and emotional distress. These are the key clinical indicators for MDD. Her limited interpersonal support, history of trauma, and social withdrawal demonstrate depressive symptoms. She has chronic hopelessness and is unmotivated by a reduced appetite and emotional void. These intrapersonal factors negatively affect her depression, affecting her physical health and diminishing her quality of life.

Complications

Without timely intervention, MDD can progress into a chronic illness. Ivy’s unmanaged MDD’s prolonged suffering, repeated relapses, and ongoing psychosocial and emotional deterioration contribute to deeper social isolation and reduced functioning (Cui et al., 2024).

Nursing Intervention

CBT (Cognitive Behavioral Therapy) and SSRIs. Encourage the development of coping strategies, promote positive thinking, and engage Ivy in improving practices.

Conclusion

MDD is a critical mental health disorder that impacts both emotional stability and physical health. Successful treatment demands an integrated strategy that includes mental health therapypsychotherapy, and medication known collectively as the 3Ps model. Utilizing this multidimensional framework enables personalized care that promotes better recovery outcomes for patients, such as Ivy Jackson. By targeting the full spectrum of MDD’s effects, healthcare experts can foster healing and elevate patients’ quality of life.
