Documentation

Accurately acquire medical information for mental health disease patients aged 18 and above in an inpatient mental health unit at the University Medical Center (UMC). Retain a record of 20 practicum hours out of 100 hours of clinical work, subject to supervisor permission—the record of patient advancement and obstacles encountered during a practicum.

Action item:  Gather preceptor’s feedback and plan clinical interventions. Create a systematic data collection process by documenting every pertinent detail on mental health problems in individuals aged 18 and up. Develop protocols regarding data entry and management. Collect and document patient outcomes, such as symptom management, readmission reduction, improved quality of life, and adherence to medicine, while maintaining adequate and constant documentation during the practicum.

Verify that all patient progress are accurately documented for further research and comparing to standard care practices.

PICOT

In individuals with mental health issue ages 18 and above (P), does implementing care coordination emphasizing interprofessional team communication (I), compared to standard care practices lacking emphasis on care coordination (C), improves symptoms management, reduce hospital readmissions, enhance medication adherence and improve patients’ quality of life (O), within six months?

Action item: Create a strategy for intervention, recruit participants, provide the intervention to individuals with mental illness, and assess the literature studies. Develop and execute the data gathering process while complying with moral standards. Monitor and contrast the results of patients undergoing care coordination intervention with those receiving standard medical care. 

Clinical Hours

Clinical hours will be spent on executing the care coordination intervention, with an emphasis on interdisciplinary communication, patient monitoring, and subsequent assessments of patient outcomes.

Action item:  Obtain clearance to do particular practicum hours. Find the total practicum hours allotted for each evaluation. Examine the patient’s needs and comprehension of psychological issues. Organize clinical hours to conduct interventions and follow-up evaluations. Confirm time is firmed up for recording patient improvement and comparing outcomes to standard care procedures.

Review

Do a thorough review of existing research over the last five years. Focusing on studies that investigate how care coordination affects individuals with mental illnesses. Review the performance and progress of care coordination intervention and impact on patients’ results like symptom management, readmission reduction, improved quality of life, and adherence to medicine.

Action item: Continued to monitor patient results and ensure consistency with project and PICOT’s objectives. Update medical documents, research pertinent articles, and assess patient outcomes to analyze progress.

Stakeholder Involvement

Involve all vital stakeholders, including psychiatrists, social workers, nurses, medical residents, medical students and psychologists to efficiently execute the intervention olan and address any issue during the process.  

Action item: Locate and involve stakeholders so that the care coordination program operates well. Regularly interact with stakeholders to keep them up to date on the project’s development and outcomes. Conduct periodic meetings with stakeholders to ensure successful cooperation and communication, evaluate advancement, and manage difficulties.
